TERMS & CONDITIONS

 

      AGREEMENT

1. This agreement is made  between  the ‘Counsellor’ and the ‘Client’ before  starting the Counselling Sessions  (called the ‘Sessions’). The Sessions aim to help the Client  achieve their  work and personal life  goals.

2. All Counsellors at Khizra  Counselling Services (the ‘counselling centre’)  follow a strict professional code  of ethics.

3.This agreement outlines the  responsibilities of both the Counsellor  and the Client.

 

                                                                                                               

           CONFIDENTIALITY

1.   Everything discussed between the Counsellor and the Client during the Sessions is strictly confidential. Counsellors may discuss  their work with their  supervisors and other professionals, but they will  not share any personal information about the Client. All  information  received by  the Counsellor is   kept confidential and follows the same ethical standards.

2. There are some exceptions to this confidentiality:

                  I. If the Client is under 18, parents or legal guardians can access records and authorize their release to a third party.
                

                  II. If the case is transferred to another counsellor.
               

                 III. If the Client is in danger of harming themselves or someone else.
               

                  IV. If the Client discloses abuse or neglect of children, the elderly, or disabled persons.
                 

                  V. If the Client needs to qualify for  certain evaluations.   

3. The practitioner does not intend for the slip to serve as a legal document. Therefore, it cannot be used as evidence in any court  proceedings.

 

             

 IMPORTANT NOTE REGARDING  CONFIDENTIALITY: 

     

 If the Client seeks medical help for  emotional difficulties, they must inform   the Counsellor. The Counsellor will only  talk to the Doctor with the Client’s   permission.

However, if there is a risk of self-harm or harm to others, the Counsellor will inform the Doctor, Police,  emergency services, or any appropriate third party. The Counsellor may consult a psychiatrist for

medical concerns about the Client.  The Counsellor may take notes during each Session, which will be stored securely. Sometimes, Sessions may be audio-recorded for monitoring purposes, but only with the

Client’s prior knowledge. These recordings will be stored securely and destroyed after use.

  SESSIONS:

1. Individual and Couples’ Sessions will not last more than 60 minutes. If the session extend beyond this, an extra 50% of the  Standard Fees will  be  charged for every additional 30 minutes.

2. The entire session time is reserved for the Client. If the Client is late, the Counsellor will adjust he appointment to avoid overlapping with the  next   Client’s session. The Counsellor will engage with the Client for the remaining time unless the  Client requests an extension. If the  Client does not arrive  within 15 minutes of the appointment time, the Counsellor will  consider the session concluded.

3. The Client must provide information that is true to the best of their knowledge. The Client is also expected to complete any outside assignments agreed  upon during the session.

4. If the Client feels any discomfort during the sessions, they should inform the Counsellor. This will help the Counsellor better   guide the session and  address the discomfort.

 5. After the first three sessions, a review will be conducted to determine if the sessions are helpful and if they should continue.

 

  FACE-TO-FACE CONTACT OUTSIDE 

 

  SESSIONS:

 

1. If the Client sees the Counsellor in a social setting, the Counsellor will not initiate any contact.  This is to protect the Client’s  privacy and confidentiality.


2. If the Client starts a conversation in such a setting, the Counsellor may respond in a professional manner.


3. If the Client and the Counsellor are part of the same social or professional group, the Counsellor has the right to withdraw  from the Client-Counsellor relationship.

 

              

 

  FEES STRUCTURE: 

A standard amount of fees is being charged by the Counselling Centre for rendering its services. The client is expected to transfer the fees 24 hours prior to the appointment time. Extra charges are levied

for Tests and additional services. It is duly stated that the charges are subjected to yearly review and in case of a change in the fees, the same shall be iterated to the Client accordingly.

 

UNDERTAKING BY THE CLIENT:

I, The Client, do hereby declare that, I have read this agreement carefully. That I have clarified and confirmed the terms and conditions as stated within this agreement.

I affirm that I understand my rights to access my personal data being held by Khizra Counselling Services.  Additionally, I give my consent to Khizra Counselling Services to use my personal and sensitive data for

the purposes of delivering Counselling Sessions to me, along with the permit to use my data for the purposes of improving and management the quality of service being delivered by Khizra Counselling Services.

 

This above mentioned agreement is fully understood and agreed to by the Client.
